For your first visit, plan to arrive 15 minutes before your scheduled appointment time. Please complete your patient forms online over our HIPAA-compliant secure server. These forms will be immediately sent to us to provide your Therapist with specifics about your medical history, injuries, pain, and current function.
- >Have your physician complete your Doctor Order or referral.
- Bring your insurance card or complete insurance information.
- Wear loose fitting clothes / Work out clothing
For your convenience, we’ve provided our patients’ forms on our HIPAA-compliant secure server, so you can complete them online and have them immediately sent to our office before your first visit. If you choose to fill out the patient form packet at our office, please arrive 10-15 minutes before your appointment to allow enough time to get them completed before your appointment.
